Complete this online registration to be entered in the Danceline State Dance Championship. An e-mail confirmation will be sent to you, along with a contest welcome packet and invoice for entry fees. Please allow 5-7 business days to process your registration.

ANITA JEFFERSON AWARD OF EXCELLENCE
Any teams entering at least one team routine are eligible for this special award. To participate in this award event, complete the appropriate space on this form.
Names not on this form will not be guaranteed an award. If no name is indicated on this official registration sheet, no award will be ordered and thus no award
given. Directors may change the name prior to contest if necessary but they may not be added later.
 
CUSTOMER AGGREEMENT
I understand that by applying for admission to a Danceline Competition, my performers and I agree to follow all rules of the competition and the site on which it is held. Danceline and/or owners of competition site will not be held responsible for any accident. I give my permission for Danceline to use performers, choreographers and teachers likeness in photographs and videos for advertising and publicity purposes. I understand that all fees are nonrefundable but may be transferred to another 2012 Danceline Competition or max. $150 to a 2012 Danceline Camp. No credits will be offered if the transfer is a change from State to Regional contest. The max credits or transfers will be honored ONLY if all fees are paid in full at least 3 weeks prior to a contest. If fees are not paid in full at least 3 weeks prior to contest no early discounts or special savings will be applied to final balances and no transfers or camp credits will be honored.
